Enrollment Requirements
The following documents should be submitted to the school before application deadline.
- Students must pass the Entrance Exam (TEAS-7), Admissions Interview & Director’s interview.
- Copy of CA Identification & Social Security Card (Copy of Passport, VISA, I-94, & I-20 if Foreign Student without Social Security).
- High School Diploma or GED & Transcripts to show GPA (If schooling was in a foreign country, it needs to be evaluated from a Foreign Educated Credentialing Agency).
- Signed Health Report from a physician stating you can participate in the program without any limitations.
- Vaccinations: Proof of Negative TB Skin test or Chest X-ray
- Tetanus, D iphtheria, Pertussis (Tdap)
- Measles, Mumps, Rubella (MMR)
- Hepatitis B (HepB)
- Influenza (Flu)
- Covid Vaccination
- A Urine Drug Screen Test (minimum 10 panel).
- A Background Check.
- A Student Liability Malpractice Insurance.
- American Heart Association Basic Life Support (CPR) card & Hospital Fire Card (Blue).
